WP Uptime Robot Monitor
This WordPress plugin lets you easily monitor your servers/websites uptime and downtime from the WordPress dashboard. You can also create your own status page using front end shortcodes provided by this plugin. It uses the uptimerobot.com 2.0 API so you don’t have to take up server resources to monitor the uptime/downtime. The uptime robot API key will give you up to 50 free monitors (websites) which can each be checked every 5 minutes which is plenty for most web admins. It supports HTTPS. Ping, Keyword, and Port checks.

Using the plugins shortcodes you can create status pages on the front end of your website easily. You can adjust the order of the shortcodes, move them onto multiple pages, have them all on one page, etc! It is very customizable and the short codes will work with any theme. Below is a few examples:
